Pre-populate Form Fields - Using IQA

Use and IQA query to pre-populate a field, or multiple fields on a form, or to display data to the user

**The 'Read-Only' option has been checked on both text box controls

Instructions

In this example, one IQA is being used to pre-populate multiple fields
The field is mapped using the file path in the document system, then a semi-colon then the field name OR the alias (column header) in the IQA.
The fields can either be written to a table in the stored procedure, can just be used as a simple display to assist the user.  If displaying the data only, do not link to the stored procedure or add a parameter name.
